RN Care Manager - Exempt

Job Summary
Assess care needs of participants and provide nursing interventions while evaluating outcomes on an ongoing basis. Collaborate with the interdisciplinary team to develop comprehensive plans of care and delegate tasks to clinic, center, and homecare aides. Monitor acute and chronic care needs across all settings, ensuring timely follow-up on specialist visits and reconciling monthly facility medication records. Provide direct care in clinics, centers, and participant homes, teaching self-care and infection control protocols. Participate in interdisciplinary team meetings, utilization reviews, and quality improvement projects. Maintain confidentiality and follow all safety and regulatory guidelines.
Required Qualifications
- State RN License
- Licensed by the New Jersey State Board of Nursing
- BLS certification
- 1 year of experience providing care as an RN
- Reliable transportation
- Valid driver's license
- Minimum state required liability auto insurance
- Medically cleared for communicable diseases
- All immunizations up to date
- Pass a comprehensive criminal background check
- Full-time employment
- Monday - Friday schedule
- 8am - 4:30pm schedule
Desired Qualifications
- Bachelor of Science in Nursing Degree
- 1 year of experience working with a frail or elderly population
- Experience in home care, long-term care and/or managed care
