Job Summary
Conduct thorough assessments of patients' medical, social, and psychological needs and collaborate with healthcare providers to develop comprehensive care plans tailored to individual patient goals. Identify barriers to care, coordinate referrals to specialists, and oversee transitions of care including admissions and discharges. Serve as a liaison between patients, families, and community resources while educating patients on self-management strategies. Maintain accurate electronic health records, prepare progress reports, and participate in quality improvement initiatives to improve care processes. Ensure compliance with all relevant healthcare regulations and standards. Work Monday through Friday from 8:00 a.m. to 4:30 p.m. with rotating weekends, including mandatory winter and summer holiday coverage. Requires a Bachelor's or Master's degree in nursing, two years of clinical experience, and an active Illinois RN license.
Required Qualifications
- Bachelors Degree Nursing
- Masters Degree Nursing
- 2 Years of clinical nursing experience
- Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) Current Registered Nurse (RN) license in the state of Illinois
- Certified Case Manager (CCM) or related field
- CPR/BLS for the healthcare provider American Heart Association (AHA)
- 1 winter and 1 summer holiday coverage
- Monday-Friday, 8:00a-4:30p
- rotating weekends
Desired Qualifications
- Masters Degree Nursing Preferred
- 2 Years of clinical nursing experience, preferably in an acute care or hospital setting
- Certified Case Manager (CCM) or related field Preferred
