Job Summary
Conduct thorough assessments of patients' medical, social, and psychological needs and collaborate with healthcare providers to develop comprehensive care plans tailored to individual patient goals. Identify barriers to care, coordinate referrals to specialists, arrange home healthcare services, and oversee transitions of care including admissions and discharges. Maintain accurate electronic health records, prepare progress reports, and educate patients on self-management strategies while ensuring compliance with healthcare regulations. Serve as a liaison between patients, families, and community resources to facilitate seamless care delivery.
Required Qualifications
- Bachelors Degree Nursing
- Masters Degree Nursing
- 2 Years of clinical nursing experience
- Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) Current Registered Nurse (RN) license in the state of Illinois
- Certified Case Manager (CCM) or related field
- CPR/BLS for the healthcare provider American Heart Association (AHA)
- 1 winter and 1 summer holiday coverage
- Monday-Friday, 8:00a-4:30p
- rotating weekends
Desired Qualifications
- Masters Degree Nursing
- 2 Years of clinical nursing experience, preferably in an acute care or hospital setting
- Certified Case Manager (CCM) or related field
