RN Care Manager Emergency Room PD Nights

Conduct comprehensive assessments of patient psycho-social, cultural, spiritual, and physical needs to identify intervention opportunities. Coordinate care progression and discharge planning across acute, extended, and post-acute settings while managing length of stay and resource utilization. Facilitate precertification, payor authorization, and collaborative management with physicians and interdisciplinary teams to ensure timely care delivery and reimbursement. Address barriers to discharge, including psychosocial and financial issues, and advocate for effective care transitions.
