Job Summary
Conduct comprehensive assessments of patient psycho-social, cultural, spiritual, and physical needs to identify intervention opportunities. Coordinate care progression and discharge planning across acute, rehabilitation, and post-acute settings while managing length of stay and resource utilization. Facilitate precertification, payor authorization, and collaborative management with physicians and interdisciplinary teams to remove barriers to timely care delivery. Evaluate outcomes, monitor readmission risks, and resolve system obstacles impeding diagnostic or treatment progress. Ensure safe transitions to community settings by addressing financial barriers, arranging durable medical equipment, and engaging support networks.
Required Qualifications
- Bachelor of Science degree in nursing
- Minimum of one (1) to three (3) years of hospital or related experience
- Internals with at least 18 months acute care case management/coordination experience
- Able to navigate and connect successfully with outside provider networks (Health Plans, IPA's, and FQHC's)
- Current California Nursing license
- Current Basic Life Support (BLS)
- Certification in Case Management
- Must complete annual Workplace Violence Prevention Program/Certificate, per hospital policy, during initial training/orientation but not to exceed 90 days from hire/transfer
