RN Care Manager - Condition/Disease Management Focus

Job Summary
Conduct case management services including assessments, care planning with prioritized goals, and care coordination across the healthcare continuum. Evaluate member needs using clinical or behavioral information, review medical test results to update care plans, and process referrals from physicians to specialty providers. Facilitate communication among providers and community resources while coaching members on self-management approaches and motivational interviewing. Educate families regarding health conditions, medications, and insurance benefits to support cost-effective outcomes. Schedule visits and participate in facility-based care conferences for transition planning. Maintain awareness of current best practices and new developments.
Required Qualifications
- Graduation from accredited school of nursing
- RN licensure in the State of Wisconsin
- Four years of clinical health care experience as a RN
- Candidates must reside in the state of Wisconsin for consideration
- Reliable internet is required
- Travel to the corporate office in Menasha is required occasionally for the position, including on first day
Desired Qualifications
- Bachelor's degree in Nursing preferred
- Case Management certification preferred
- Chronic condition management experience preferred, especially: Diabetes, Cardiac, Pulmonology
- Previous experience in case management, utilization management, insurance, or managed care preferred
- Experience with Medicare, Medicaid preferred
