Job Summary
RN Care Manager in Cleveland responsible for coordinating and evaluating the management of patients with acute and chronic conditions across the care continuum to achieve high-quality outcomes. Develops and executes comprehensive, individualized care plans with the care team, supports transitions of care from centers to acute/post-acute facilities and home, provides patient education and care coordination, and uses motivational interviewing to engage patients. Collaborates with clinical staff, reduces unnecessary referrals, and ensures regulatory compliance and quality care. Requires RN license, associate degree (BSN preferred), 2 years of clinical experience, and experience in utilization review/discharge planning/case management; CCM certification is preferred or obtainable within a set timeframe; Spanish or other language skills are a plus.
Required Qualifications
- Associate degree in Nursing required
- RN license active in state of employment
- Bachelor’s Degree in Nursing (BSN) or RN with bachelor’s degree in related clinical field preferred
- Minimum of 2 years clinical work experience
- Minimum of 1 year of utilization review, home health, discharge planning experience highly desired
- Minimum of 1 year of case management experience in acute or ambulatory case management highly desired
- Certified Case Manager certification preferred or willingness to obtain within specified time
- ICD-10, CPT codes, HCPCS knowledge preferred
- Managed care experience including knowledge of HEDIS and CMS guidelines preferred
