RN Care Manager

The RN Care Manager (RNCM) manages a panel of rising and high intensity members to support integrated chronic disease and behavioral health care. Responsibilities include creating and overseeing care plans, conducting clinical assessments, collaborating with care teams, participating in case conferences, and supporting members in achieving their care goals. The RNCM must have strong skills in case management, clinical support, medication reconciliation, and use of electronic health records. Candidates are required to have a minimum of 3 years of experience and must be a graduate of an accredited nursing program.
