Job Summary
Conduct case management, utilization review, and care planning to guide high-risk patients with chronic conditions through the continuum of care. Coordinate social service integration and ensure compliance with quality patient care and regulatory standards. Collaborate with clinical staff to develop care plans, verify appropriate lab testing, and promote effective resource utilization. Engage patients and families in self-management and informed decision-making while adhering to Population Health Management protocols. Requires Arkansas RN license and 2+ years of case management experience; Monday-Friday 8:00 AM-5:00 PM shift at Community Clinic Primary & Specialty Care Clinic.
Required Qualifications
- High school diploma or equivalent
- Arkansas RN License
- 2+ years of Case/Care Management experience
Desired Qualifications
- 1+ year(s) of Rheumatology experience
- 2+ years of utilization review/case management experience
- 1 year of experience in discharge planning/hospital
- 1 year of experience in primary care
- Knowledge of clinical practices in an ambulatory care, family practice setting
