RN Care Manager - Care Coordination

Facilitate payor authorization processes and ensure accurate patient type assignments while managing collaborative care across the continuum. Work with the CDI team to verify documentation severity and maintain active communication with physicians and the multi-disciplinary care team to optimize discharge and length of stay. Address system problems impeding treatment progress and seek consultation from appropriate departments to expedite care delivery. Monitor patient progress to ensure plans of care remain patient-focused, high quality, and cost-effective. Requires current IL nursing licensure, a Bachelor's degree, and minimum two years of clinical nursing experience. Full-time role at Endeavor Health in Arlington Heights, IL, with rotating weekend coverage and holiday requirements.
