RN Care Manager - Care Coordination

Facilitate payor authorization processes and ensure accurate patient type assignments. Manage patient care across the continuum by collaborating with physicians, nursing, and multi-disciplinary teams to remove barriers to timely discharge and optimize length of stay. Work with the CDI team to verify documentation severity and intensity, addressing system problems that impede diagnostic or treatment progress. Monitor patient progress to ensure plans of care remain patient-focused, high quality, and cost-effective. Requires current Illinois nursing licensure, a Bachelor's degree, and at least two years of clinical nursing experience. Full-time role in Arlington Heights, IL, with rotating weekend coverage and holiday requirements.
