RN Care Manager - Care Coordination

Facilitate payor authorization processes and ensure accurate patient type assignments while managing patient care across the continuum to optimize discharge and length of stay. Collaborate with physicians, nursing, and the CDI team to resolve system barriers, verify documentation severity, and expedite care progression. Monitor patient progress to ensure high-quality, cost-effective plans of care, proactively identifying and resolving obstacles to timely treatment. Requires current Illinois nursing licensure, a Bachelor's degree, and minimum two years of clinical experience, with preference for Utilization Management or chronic disease management backgrounds. Full-time role with rotating weekend coverage and holiday requirements at Endeavor Health's Arlington Heights location.
