Job Summary
RN Care Manager position focusing on senior patient care across the care continuum, including in-person, video, and remote visits. Responsibilities include prevention (Annual Wellness Visits), medication reconciliation, geriatric assessment, care plan reinforcement, chronic disease follow-up, and advance care planning. Collaborate with primary care providers and health navigators to coordinate follow-up care, attend case conferences and team huddles, and coordinate with community resources (home health, durable medical equipment vendors, specialists) to deliver high-quality, whole-person care. Identify and act on open care gaps (cancer screenings, hearing/vision assessments, metabolic health, vaccines, functional status evaluations, fall risk, cognition/mental health screenings) and provide clinical support for Health Navigators. Required RN licensure in CA; willingness to obtain licenses in other states; 2+ years nursing experience; BSN or MSN preferred. In-office role located in Strawberry Village with ability to commute to San Rafael; standard hours 8am-5pm PT. Required Qualifications
- Licensed RN in CA required
- Willingness to obtain licenses in other states
- Bachelor of Science in Nursing or MSN preferred
- 2+ years of nursing experience in hospital, post-acute facility, or ambulatory setting
- Working in office with primary care providers and health navigators
- Must be able to work in office hours 8am-5pm Pacific Time
- Ability to conduct in-person, video, and remote nursing visits
- Experience with care management, value-based care, or population health preferred
- Must demonstrate strong interpersonal and communication skills (in-person, telephone, video, and electronic)
- Must be able to think autonomously and manage changing environments
