Plan and prioritize care for individuals and populations, developing care plans and making recommendations to providers to maximize continuity across the continuum. Assess patient needs, insurance benefits, and resources while assessing educational requirements for patients, families, and the health care team. Coordinate services with providers and payers to improve access and promote optimal health in a cost-effective manner. Measure patient care outcomes, interpret reports, and analyze data trends for groups of patients. Document patient data, interventions, and outcomes according to department guidelines.
