Job Summary
RN - Care Manager role in the New York Tri-State Area (NY, NJ, CT) requiring 3+ years in Managed/Home/Community care. Hybrid full-time position with a NYS RN license, an Associate's degree or higher, and bilingual capabilities. Responsibilities include care planning, coordination, health assessments, monitoring service quality, coordinating transitions and discharge planning, and collaboration with an Interdisciplinary Care team to address health, environmental, and psychosocial risks. Competitive salary ($95k-$105k) and comprehensive benefits package; remote-hybrid work arrangement in the assigned tri-state area.
Required Qualifications
- Valid NYS RN License
- Bachelor's degree preferred but at least Associate's degree required
- Minimum 3 years of relevant experience in Managed Care, Home Care, or Community settings
- Bilingual proficiency with specified languages (e.g., Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, Creole)
- Strong communication, problem-solving, and software/tool navigation skills
