Job Summary
Assess patients on admission for family support, insurance resources, and discharge planning needs. Engage in daily multidisciplinary rounds Monday through Friday to develop and implement discharge plans with input from various disciplines. Coordinate post-acute needs in a timely, cost-efficient manner while working with payors to obtain authorizations for ongoing hospitalization or identified post-acute care. Collaborate with patients, families, community resources, and agencies to ensure appropriate levels of care. Lead multidisciplinary and family care conferences for complex situations. Comply with regulatory requirements of DNV, Medicaid, Medicare, and the Oregon Nurse Practice Act.
Required Qualifications
- Minimum three (3) years of clinical nursing experience in the last 7 years
- Minimum one (1) year of Care coordination/transitions of care, charge nurse, home health, and/or hospital at home RN experience
- Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date
- Associate Degree in Nursing from an accredited program 30 days before the start date
- Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire
- Current, unencumbered Oregon State Registered Nurse License
- BLS from AHA required
- Must be able to perform the essential functions of the position with or without accommodation
Desired Qualifications
- Case Management Certification (ACMA, RN-BC, CMSA)
- Hospital based case management experience
- Proficiency in managing difficult and sensitive family and provider care conferences including adjustment to new levels of function, end of life discussions, imbalance of available resources (social, spiritual, medical, financial)
- Progressive leadership positions within a nursing unit, examples include Charge nurse, UBNPC Chair/Member, Shared Governance councils
- Minimum two (2) years of Inpatient Case/Care Management experience in the last 5 years
