RN Care Coordinator - Part Time

Job Summary
Review and oversee nursing and home health aide services, including patient admission assessments, care planning, goal setting, evaluation, coordination of care, emergency/safety plans, and discharge. Participate in training new and existing personnel with the Director of Home Health, ensure timely follow-up with hospitalizations, order supplies, and communicate with the interdisciplinary team. Perform accurate data entry in electronic medical records and collaborate with outside providers to maintain reliable patient care communication. Maintain compliance with county, state, federal regulatory initiatives and Accurate Home Care policies. Location: Otsego Office and Remote. Hours: 8-10 hours/week. Salary: $37.00-$41.00 hourly. Requires Minnesota RN license and one-year Medicare home health experience preferred.
Required Qualifications
- Must be licensed as a Registered Nurse (RN) in Minnesota
- Maintain confidentiality in accordance with applicable laws and rules
- Ability to read, write speak fluently in English
- Confirmed negative tuberculosis status
- Effective communication, interpersonal and critical thinking skills
Desired Qualifications
- One-year Medicare home health visit nurse experience preferred including proficiency with OASIS
