Job Summary
RN Care Coordinator for Outpatient Anesthesia Pain Management at Cleveland Clinic Neurological Institute/Cerebrovascular Center. Full-time with a hybrid arrangement after 90 days (2 days on-site, 3 days work-from-home). Responsibilities include collaborating with a multidisciplinary care team to develop goals and interventions, disease and care coordination, comprehensive clinical assessments covering medical, age-specific, and end-of-life needs, patient and family education and coaching, monitoring adherence to care plans, reassessments, liaison and advocacy, supporting transitions of care, identifying barriers and connecting patients with community resources. Minimum qualifications: RN license, BLS certification, and 3-5 years of nursing experience. Preferred: BSN and specialty certification.
Required Qualifications
- Graduate from an accredited school of professional nursing.
- Current state licensure as a Registered Nurse (RN).
- Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross.
