Job Summary
Provide disease management support through proactive monitoring, assessment, education, and follow-up for patients with chronic conditions. Deliver competency-based education to patients, families, staff, and providers while maintaining care management databases. Participate in cost-effective service control by ensuring appropriate staff levels, equipment use, and supply management. Evaluate and implement clinical program components to optimize patient care strategies and goals. Demonstrate effective interviewing and assessment skills to plan, implement, and evaluate patient care plans. Deliver timely internal and external customer service in a cooperative, professional, and respectful manner.
Required Qualifications
- Must be licensed as RN for two (2) years
- Valid RN license
- Knowledge of evidence-based clinical guidelines across a wide variety of conditions and age groups
- Knowledge of care management and resource/utilization management
- Skill in the use of personal computers and related software applications
- Ability to manage multiple priorities under time constraints
- Ability to analyze and solve problems
- Understanding cost and quality issues
- Verbal and written communication skills
- Interpersonal skills to interact with a wide range of constituencies
- Decision-making skills
- Knowledge of Order to Cash
- Knowledge of Procure to Pay
- Knowledge of Supply Chain
- Knowledge of Customers
- Knowledge of Suppliers
Desired Qualifications
- RN experience in clinical nursing with chronic disease, utilization review and/or quality management
