Job Summary

Assess and identify patient and family needs to coordinate the Plan of Care with the Interdisciplinary Team (IDT) and provide clinical, palliative, and supportive care to keep participants in their home environment. Conduct home visits to evaluate safety, medication, nutritional, and DME compliance while educating families on disease processes and palliative interventions. Manage direct care by supervising Community Health Workers and Home Health Aides as requested, reporting condition changes to the IDT, and initiating skilled nursing interventions to alleviate symptoms. Complete initial and semi-annual assessments, obtain physician orders, and document all contact information in the EMR. Represent the company professionally at assigned facilities, facilitate referrals, and attend daily IDT collaboration meetings. During emergencies, report to designated locations to ensure service continuity.

Required Qualifications

  • Current license as RN in the state where the employee will be working
  • Minimum of one (1) year nursing experience
  • Valid driver's license
  • Automobile insurance per Company policy
  • Reliable transportation to meet visit schedule
  • Ability to use equipment with visual and auditory mechanisms
  • Ability to effectively communicate in English (verbal and written)
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross
  • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation
  • Consent to drug and/or alcohol testing after a conditional offer of employment is made
  • Ongoing compliance with the Drug-Free Workplace Policy
  • Submission through the Florida Care Provider Background Screening Clearinghouse to verify eligibility

Desired Qualifications

  • Hospice or hospital experience
  • Employees working at PACE, certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs
  • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
  • Ability to visit Participant in their homes to assessments
