RN Care Coordinator - LTC

Licensed RN Care Coordinator responsible for resident-centered care and regulatory compliance across Juliette Manor and Peabody Manor. Supports RN Clinical Managers in planning, organizing, and directing nursing services; coordinates schedules to meet nurse-to-resident ratios and budget goals; mentors nursing staff, participates in interview/onboarding, and provides ongoing education and leadership. Ensures regulatory survey readiness, participates in QAPI activities, collaborates with providers and interprofessional teams to problem-solve and improve resident care. Manages admissions assistance, care planning, MDS processes, and documentation with residents and families. Requires clinical expertise in nursing duties, knowledge of CMS/state regulations, EMR proficiency, and ability to lift up to 50 pounds.
