Job Summary
RN Care Coordinator at Partnership Health Center (Somerville, NJ) manages a full-time on-site role focused on coordinating care for chronically ill/high-risk patients. Daily activities include engaging 10-15 patients per workday through face-to-face visits, telephonic outreach, and collaboration with treating providers; activities also cover documentation research, analysis, and care planning. A typical day splits 30-45 minutes of extended patient meetings with the remainder spent on follow-up with patients, family/caregivers, providers, and community resources via secure messaging and calls. Required Qualifications
- RN license/registration
- Graduation from an accredited university or community college with a background in science or nursing (e.g., Nursing, Biology, Chemistry, Public Health)
- 5-10 years of experience in Hospital, Ambulatory Care, Post-Acute Care, Health Plan/Benefits Management, and Disease Management
- Strong written and verbal communication skills
- Ability to maintain confidentiality according to State and Federal regulations
- Proficient computer skills including Microsoft Office (Word, Excel, PowerPoint, Access)
- Ability to engage 10-15 patients per day via face-to-face visits, telephonic engagement, and engagement with treating providers
- Team orientation and ability to follow systems and protocols
- Demonstrated judgment, decision-making and problem-solving skills
- Experience coordinating with community resources and healthcare organizations
