Job Summary
Manage EHR tasks including telephone encounters, lab follow-ups, and notifying patients of test results. Coordinate care by reviewing schedules, checking immunization status, and ensuring external hospital records are up to date. Answer triage calls and serve as the primary contact for Mission Control to schedule provider visits. Assist with non-medication prior authorizations for DME, diabetic shoes, and diagnostic imaging. Participate in daily care team huddles and maintain continuous communication between providers and patients. This remote RN Care Coordinator supports the Diabetes Program and primary care nursing team to ensure quality, continuous care for diverse patients.
Required Qualifications
- Possession of a valid license as a Registered Nurse in the state where nursing practice will occur
- Multistate license in nurse's resident state
- Two years nursing experience in an outpatient clinic
- Basic Life Support (BLS) certification through American Heart Association
- Social and cultural sensitivity appropriate to ethnically and economically diverse patient-and employee-base
- Demonstrate knowledge of the rationale of appropriate patient care
- Must be organized and able to manage/prioritize tasks to meet deadlines
- Adapts to change in a positive manner
- Able to work as a part of a team
- Communicates through appropriate channels
- Use proper chain of command for patient complaints
- Ability to handle emergency situations calmly and effectively
- Must be computer literate, especially with Microsoft Office products
- Be proficient with the Electronic Health Record software
- Maintain good inter-personal relationship with co-workers and other members of the health care team
- Provide customer service in accordance to the organization's mission
- Be courteous and respectful when interacting with patients and family members
- Maintain patient confidentiality in accordance to organization's policy and procedure and HIPAA requirements
