Job Summary
RN Care Coordinator role in Augusta, GA requiring an Associate's in Nursing, BLS, and an active RN license (GA or multi-state). Responsibilities include guiding and educating patients and families, health management and care coordination for a designated patient population, and addressing barriers related to medical needs, insurance, health literacy, and socioeconomic factors. Preferred: Bachelor's in Nursing, 1+ year in Case Management, national Case Management certification. Required Qualifications
- Associates Degree in Nursing
- 2 years or more of experience in Nursing
- Current American Heart Association or American Red Cross Basic Life Support (BLS) certification
