Job Summary

Make after-hours field visits to patients, families, and community partners at homes, assisted living facilities, nursing homes, or hospitals. Assess immediate needs and provide direct care including intravenous therapies, pain medication titration, and symptom management within an interdisciplinary group. Educate patients and families on disease processes, palliative interventions, and safety practices while managing referrals and coordinating with physicians. Complete clinical admissions, informed consents, and documentation per policy; prepare bodies for transport and provide bereavement support. Report condition changes to the team and facilitate safe discharges during emergencies if required.

Required Qualifications

  • Current license as RN in the state where the employee will be working
  • Minimum of one (1) year nursing experience
  • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
  • Ability to effectively communicate in English (verbal and written)
  • Valid driver's license
  • Automobile insurance per Company policy
  • Reliable transportation to meet patients' needs
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross
  • Ability to perform sedentary work with frequent interruptions
  • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodations
  • Ability to work in a cooperative team environment
  • Ability to exhibit a sense of urgency in daily work activities
  • Skilled in computer operations
  • Proficiency in MS Office software applications
  • Proven data entry and navigation skills
  • Available to work shifts to accommodate extended hours of operation as scheduled

Desired Qualifications

  • Hospice or hospital experience
  • Bi-lingual in Spanish
  • Ability to effectively handle stressful situations professionally and calmly with exceptional customer service
  • Strong patient service skills
  • Information seeking / probing skills
  • Listening, understanding, and responding
  • Ability to navigate in a fast-paced environment utilizing interpersonal skills during patient visits
  • Ability to manage the emotional stress of working with and caring for terminally ill patients and their families
