RN - Cardiology Watchman Coordinator

Develops and implements individual plans of care in collaboration with the healthcare team while assessing patient health status and evaluating responses to interventions. Coordinates care across the continuum including outpatient procedures and cath lab orders, managing patients with chronic conditions and acute episodes. Promotes patient-centered care within an ambulatory setting by executing nursing interventions and assisting in clinic operations. Floats to other clinics as needed for operational requirements.
