Job Summary
Provides skilled nursing care to patients in their homes under physician orders and plan of care, including initial evaluation, development and revision of the plan of care, coordination with IDT and physicians, supervision of LPN/LVN and HHAs, patient and family counseling, participation in in-service programs and quality management, and completion of documentation and OASIS assessments; requires licensure in the practicing state, graduation from an approved nursing program, and ability to exercise independent judgment; on-call coverage evenings, weekends, and holidays; location includes Meridian, Idaho, with related home care services.
Required Qualifications
- Graduate of an approved school of professional nursing
- Currently licensed in the state(s) in which practicing
- One year nursing experience, preferred
- Acceptance of philosophy and goals of this Agency
- Ability to exercise initiative and independent judgment
- Provides services in accordance with the plan of care
- Completes and submits OASIS assessments, reassessments, and transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames
- Maintains CPR certification
- Takes on-call duty nights, weekends and holidays as assigned
