Job Summary
Answer incoming phone calls from patients and families, communicate information to the primary RN or team members as required, and redirect nursing visits. Maintain accurate Patient/Family profiles including medication data, manage physician orders, and follow up on lab results. Assist with home visits, admissions, record reviews, and forwarding referrals to massage and music therapists. Hold strictest confidence on all patient information and disclose data only to authorized Hospice personnel. Complete mandatory training, attend staff meetings, and utilize agency technology to increase efficiency. Licensed driver with dependable, insured automobile required.
Required Qualifications
- Graduate of an accredited school of registered nursing
- Currently licensed as an RN through the Ohio Board of Nursing
- Minimum 2 years of clinical experience in acute setting
- Has working experience charting in an EMR system
- Possesses clinical expertise related to critical thinking, assessment, problem-solving, and technical skills
- Possesses the ability to provide basic and skilled nursing care, set priorities of care, and appropriately assess limits of intervention
- Possesses strong verbal and written communication skills
- Has the ability to work independently and within an interdisciplinary team
- Demonstrates strong organizational, time management, and prioritization skills
- Understands the hospice philosophy, principles of death/dying, concepts of pain/symptom management, and adult teaching/learning principles
- Must be a licensed driver with dependable automobile that is insured in accordance with state and/or organization requirements
- Must be CPR certified according to the American Heart Association CPR standards
