Job Summary
Registered Nurse (RN) for a Full-Time Home Health role in Bellefontaine, OH. Responsible for planning, organizing, and directing home care services with emphasis on community health education; conducts initial patient and family assessments, develops and executes care plans, administers medications, provides direct patient care, and coordinates patient care with the physician and care team. Performs patient and family education, discharge planning, and acts as case manager as assigned. Requires on-call duties, travel within ~30-mile radius, mileage reimbursement, and possible remote/office-based training. Must have nursing degree, active license, CPR, TB test, and valid driver’s license; prior home health or acute care experience preferred; strong communication and problem-solving skills; ability to lift and assist patients as needed. Required Qualifications
- Graduate of an accredited school of nursing (associate or bachelor’s degree)
- Valid/active nursing license
- Must be a licensed driver with an automobile in good working order and insured
- Valid CPR certification
- Recent TB test
