Job Summary
RN Assessor coordinates care with interdisciplinary teams, participates in treatment planning including psychosocial assessments, provides warm-referrals to community providers, completes thorough documentation in compliance with standards, demonstrates crisis intervention skills, and participates in admissions including benefits verification and necessary authorizations. The role involves night shifts (7p-7a) with a schedule of 3 12-hour shifts per week, and offers Full Time, Part Time, and PRN opportunities at ClearVista Behavioral Hospital in Lorain, OH; team-based care for adults and seniors with behavioral health needs and a focus on collaboration with clinicians to develop and oversee individualized care plans.
Required Qualifications
- RN (Registered Nurse) required
- Bachelor's degree (LSW or RN preferred) or Master's degree in social work or counseling with licensure
- Licenses: BSW, LSW, LISW, LPC, LPCC, MFT
- 21 years old or older
