Job Summary
Conduct initial and annual health assessments promoting health, safety, dignity, and respect for individuals served. Collaborate to develop person-centered care plans while educating clients and families on relevant health matters. Maintain relationships with funding sources and stakeholders, and promote InstED emergency department diversionary services. Complete documentation, forms, and Critical Incident reporting per policy; attend in-services and mandatory trainings. Maintain familiarity with One Care policies and procedures. Requires current Massachusetts RN licensure, valid driver's license, and experience with behavioral health and substance use disorders. Salary range $40-$46/hr.
Required Qualifications
- High School Diploma or GED
- Current Registered Nurse License by the Massachusetts Board of Registration

- Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance use, and care management

- Demonstrates cultural and linguistic competence in working with individuals of varied cultural backgrounds and linguistic needs
- Exemplary and culturally sensitive customer service skills directed toward individuals served
- Demonstrates willingness and ability to serve challenging and diverse populations
- Familiarity with Internet, email systems, and electronic health records (EHRs)
- Strong verbal and written communication skills
- ability to communicate clearly and effectively with individuals served, providers, and staff
- Working knowledge of program KPIs and performance metrics and how they impact the role, the program, and the overall mission of the One Care program
Desired Qualifications
- Demonstrated experience providing services to individuals with behavioral health and substance use disorders
- Bilingual Spanish and English speaking
