Registered Nurse needed at All Home Care Services to perform comprehensive assessments (vital signs, physical examinations, patient history), administer medications and IV fluids with monitoring and documentation, and develop and implement individualized care plans. Collaborate with the interdisciplinary team to meet patient needs, provide patient education and discharge planning to support self-management, communicate with physicians, other healthcare providers, and patients/families, maintain accurate records of assessments and interventions, and contribute to a supportive, growth-focused environment for caregivers.**Responsibilities include** conducting assessments, medication administration, plan development, care coordination, patient and family education, and thorough documentation.
