RN - Adult Medicine Clinic (36 hours/week)

Conduct patient assessments, provide education, and schedule appointments via telephone and in-person triage at the Heart of the City adult medicine clinic. Advise patients and parents on care plans, ensure follow-up call-backs, and coordinate services with other health agencies. Document nursing actions in medical records, participate in interdisciplinary planning, and contribute to policy development and quality assurance. Provide translation services when needed while adhering to organizational compliance standards and CSHS regulations. Work onsite Monday through Friday with occasional Saturday mornings, supporting Cherry Health's mission to expand access to comprehensive care for underserved communities.
