Job Summary
Complete clinical admissions for all Company services by assessing patients in their place of residency or hospitals and initiating care into the Chapters Health System. Explain services to potential patients and families, address immediate needs, obtain informed consents, and initiate skilled nursing interventions to alleviate symptoms. Provide information to physicians, obtain orders, and develop strong relationships with referring hospitals, physicians, and case managers. Communicate frequently with Care Navigation staff to ensure timely follow-up, discuss potential needs with after-hours staff, and prepare for the next assignment. Schedule includes 4-10 hour shifts and every other weekend.
Required Qualifications
- Current license as RN in the state where the employee will be working
- Minimum of one (1) year nursing experience
- Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
- Strong clinical assessment skills
- Active BLS for healthcare professionals from the American Heart Association or Red Cross
- Valid driver's license
- Automobile insurance per Company policy
- Reliable transportation to meet schedule
- Ability to effectively communicate in English (verbal and written)
- Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation

Desired Qualifications
- Hospice or hospital experience
- Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
- Background in emergency department or ICU
- Bi-lingual in Spanish
- Ability to present services in a professional and articulate manner
- Strong goal/outcome orientation with exceptional time management skills
- Excellent customer service focus
