RN Admissions

Coordinate referrals, answer admission criteria questions, and process paperwork for hospice patients and families. Record information on tracking sheets, contact physicians for approval, and schedule admission interviews within 24 hours. Obtain written consent when criteria are met and communicate with referral sources regarding appointments, medical orders, and time frames. Provide accurate documentation of services while working with the marketing team to ensure patient satisfaction and maximize census goals. Evening shift from 2pm to 10pm. Registered Nurse license and hospice experience preferred.
