Job Summary
Assess patients in their place of residency or hospitals and initiate care into the Chapters Health System services. Ensure a seamless transition to Care Navigation while acting as an ambassador of the Organization. Explain services to potential patients, address immediate needs, and complete informed consents and initial assessments according to regulations. Obtain physician orders, initiate skilled nursing interventions, and provide necessary clinical communication using SBAR. Obtain medical record information for referrals, develop relationships with case managers and physicians, and arrange safe discharge for eligible patients. Complete clinical admissions for all Company services including daily visits, professional management, and efficient discharge for GIP patients in a hospital. Communicate frequently with Care Navigation staff to ensure timely follow-up and inform the team when assignments are complete.
Required Qualifications
- Current license as RN in the state where the employee will be working
- Minimum of one (1) year nursing experience
- Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
- Strong clinical assessment skills
- Ability to effectively communicate in English (verbal and written)
- Valid driver's license
- Automobile insurance per Company policy
- Reliable transportation to meet schedule
- Active BLS for healthcare professionals from the American Heart Association or Red Cross
- Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation

Desired Qualifications
- Hospice or hospital experience
- Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
- Background in emergency department or ICU
- Bi-lingual in Spanish
- Ability to present services in a professional and articulate manner
- Strong goal/outcome orientation with exceptional time management skills
- Excellent customer service focus
