Job Summary
Admission Nurse role focused on evaluating and coordinating care for home health patients, traveling to patients’ homes within the branch coverage area, developing and revising plans of care, communicating with care teams, and ensuring quality patient outcomes; requires RN nursing degree and an unrestricted RN license, with travel up to 50% and a preference for an Associate or Bachelor's degree in Nursing, plus Home Health/Hospice experience and familiarity with Homecare HomeBase.
Required Qualifications
- Graduate of an accredited nursing program
- Current, unrestricted state RN license
- 1+ year experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Must have dependable vehicle, valid driver’s license and current auto insurance
- Must be able and willing to travel 50%
- Associates Degree or Bachelors Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with Homecare HomeBase is preferred
