Job Summary
Implement individualized restorative care plans to support residents' rehabilitation goals through mobility training, range-of motion exercises, and activities of daily living. Assist with transfers, bathing, grooming, and feeding while promoting independence and functional improvement. Observe progress, document services, and communicate status changes to the nursing and therapy team. Collaborate on care plan updates and workload scheduling. Required Qualifications
- Certified and state tested as a Certified Nursing Assistant in the state employed
- certificate must be active, valid, and in good standing
