Identify and prevent high-risk resident decline, develop attainable goals with families, and audit high-risk residents for falls to ensure interventions like floor mats and alarms are in place. Review medical equipment for safety and integrity, assist with meeting mobility objectives, and participate in Med B/CMI rounds and risk meetings weekly. Provide monthly fall prevention audits, updates to the Director of Nursing on resident progress, and accurate restorative minutes.
