Job Summary
Provide intermittent, regular scheduled, temporary care and supervision for a client with developmental disabilities residing with a family member or care provider. Assist with feeding, dressing, toileting, bathing, grooming, and light meal preparation while following specific instructions from parents or guardians. Complete daily notes, submit time cards and mileage reports using UCPIE case management software, and maintain current emergency call forms. Work a minimum of two hours per appointment, adhering to safety precautions and never leaving the client unattended. Maintain CPR and First Aid certification, operate a vehicle for daily travel to home settings, and continue education through required training modules.
Required Qualifications
- High school diploma, GED certificate
- 2 years' experience working with individuals with developmental disabilities (experience can be caring for a family member with a disability)
- Strong written and oral communication skills
- Computer proficient
- CPR & First Aide Certification
- Clean TB test every 4 years
- Valid Driver's License
- Valid vehicle registration
- Valid vehicle insurance
- Clear security clearance (fingerprint live scan) through the FBI and DOJ
- Legally eligible to work in the U.S.A.
- Must be able to lift, pull, push, run and manipulate/use adaptive equipment and patients, which requires strength and gross motor and fine motor coordination
- Must be able to perform CPR and administer First-Aid
- Must be able to operate an automobile and a computer
- Must be able to climb stairs and provide services in the family's home environment
- Must be able to work a minimum of two hours at each respite appointment
- Must never leave a client(s)/ consumer(s)/ siblings unattended
- Must adhere to policies and procedures in employee handbook and respite fundamental training manual
- Must adhere to UCPIE's Injury and Illness Prevention Plan and all other applicable safety procedures
- Must report suspected incidents of abuse to the Respite Program Manager & complete required documentation
- Must report any changes in personal information to respite program manager immediately
- Must complete and submit time cards, mileage, reports, etc. using UCPIE time keeping and case management software
- Must complete client daily notes and submit to UCPIE administrative offices as instructed by respite program manager
- Must maintain current information on emergency call forms and submit copy to UCPIE office
- Must return phones calls to UCPIE administrative team within 24 hours or immediately if emergency message is received
- Must not transport client(s) or siblings at any time
- Must continue education through training modules; workshops; annual UCPIE in-house training, etc.
- Must be available for daily travel to client's home; note travel will not exceed 40 miles unless approved by Respite Program Manager
