Job Summary
Provide case management support to the medical respite program serving individuals who are homeless or in transitional housing. Responsibilities include preparing patients for intake, collecting patient histories and vital signs, coordinating with medical staff and housing partners, documenting information in HMIS and health-care records, arranging patient services, assisting with follow-up and referrals, managing supplies, ensuring sanitation standards, and collaborating with housing case managers to transition clients to stable housing or alternative arrangements. Must have an Associate's degree (or higher) in behavioral/health sciences or related field and two years of related experience, CMA/CNA certification or willingness to learn, and a valid WA driver’s license. Bilingual Spanish/English is desirable.
Required Qualifications
- Associate's degree in behavioral or health sciences or related field and two years closely related work experience
- Experience as a medical assistant or CMA/CNA completion or willingness to learn
- Knowledge of effective case management practices; knowledge of community resources and experience in effective social service delivery
- Proficiency in computer operating systems
- Valid Washington State Driver's License and insurance
- Good written and verbal communication skills
