Job Summary
The Respite Care Program Assistant provides client coordination and support for homeless adults discharged from hospitals, ensuring wellness through monitoring and arranging services. Duties include assisting with client intake and medication management, coordinating care with a multidisciplinary team, conducting regular client observations, and participating in case conferences. Qualifications include a high school diploma, CPR certification, two years of relevant experience, proficiency in computer programs, and strong communication skills. The role requires a proactive work ethic and sensitivity to diverse populations.
Required Qualifications
- Current CPR certification and First Aid from an approved program
- Possession of a Community Health Worker certificate or CNA level certification
- At least two years of verifiable work experience working with a culturally diverse population performing various duties
- High school diploma or GED equivalent
- Proficiency in Word, Excel and data entry programs
- Ability to navigate the Internet
- Excellent verbal and written communication skills
- Strong organizational skills
- Experience and commitment to working with homeless individuals
- Sensitivity and experience working with a diverse client population
- Proactive work ethic
- Ability to prioritize and handle multiple tasks
- High degree of professional integrity
- TB clearance
- Drug-free and sober
