Job Summary
Assist patients with transportation arrangements for medical appointments and help locate affordable housing and accommodations. Research and maintain up-to-date community resource information while collaborating with social workers and community agencies to coordinate patient care. Educate patients and families on available resources and organize informational materials about support services. This role supports Mass General Brigham's mission to provide exceptional care by ensuring every patient receives the resources they need for better health and well-being.
Required Qualifications
- High school diploma

- 2-3 years in healthcare, social services, or a related field
- Bilingual Spanish
