Job Summary
Resource Specialist supports the Population Health Case Management Team to maximize hospital resource utilization, coordinate post-hospital care, and promote health equity by communicating with payers, outside agencies, and hospital departments to ensure coordinated patient care and timely post-discharge transitions. Responsibilities include preparing paperwork aligned with population health goals, faxing and chart handling, documenting in Cerner and naviHealth, initiating advanced directive discussions, scheduling and tracking care conferences, coordinating with social workers and transportation for post-discharge needs, verifying insurance post-discharge options, reviewing post-hospital services (SNF, DME, Home Health, etc.), maintaining resource updates, attending daily huddles, and producing discharge-related correspondence while ensuring adherence to Joint Commission, CMS, and DoH standards and contributing to reduced readmissions and improved population health outcomes.
