Job Summary
The Resource Specialist supports an integrated primary care and behavioral health team by connecting patients to social resources (housing, transportation, food, utilities, employment, child care, and more). Responsibilities include meeting with patients to determine social determinants of health needs; providing referrals to government and community-based organizations; educating patients on available healthcare and social services; assisting with referral forms; liaising with resources and providers to advocate for patient needs; engaging patients via technology; administering surveys and screens; processing forms for resource requests; maintaining and updating a resource tracking database; generating utilization reports; maintaining partnerships with community resources; participating in team trainings and meetings; coordinating with student interns to delegate/triage referrals. Skills emphasized include excellent interpersonal and written/verbal communication, relationship-building with patients and community organizations, problem-solving, cultural sensitivity, confidentiality, ability to manage multiple priorities, adaptability to new software, and Spanish proficiency preferred.
Required Qualifications
- High school diploma or equivalent; Associate Degree or Bachelor’s degree in health sciences, community health, social work, psychology, public health is preferred but not required.
- Community Health Worker certification preferred but not required.
- Experience working in a health care setting preferred; medical terminology knowledge
