Assess, plan, implement, and evaluate holistic patient care utilizing The Christ Hospital Professional Practice Model, including physical, psychosocial, and spiritual needs. Perform and delegate nursing actions based on physician orders, respond to emergency situations with controlled precision, and implement measures to prevent disease transmission. Document the nursing process, plan of care, and patient responses accurately in the electronic medical record. Provide ongoing individualized education to patients and families, collaborate with multidisciplinary teams using SBAR, and initiate discharge planning. Participate in 75% of staff and council meetings, contribute to performance improvement projects, and act as a resource, educator, and preceptor for staff and students. Maintain competency in point-of-care testing and complete all required Healthstream education on time.
