Job Summary
Coordinate the guest's initial Plan of Care at admission and assist with planning, implementation, and evaluation of the guest care area/unit orientation. Utilize material, equipment, and time in a safe, beneficial, and cost-effective manner while working within the interdisciplinary team structure. May perform other duties as assigned. This role requires a current LPN or RN license with at least three years of experience in an LTC/SNF/rehab setting, plus CPR certification and fingerprint clearance.
Required Qualifications
- A graduate of an accredited school of nursing
- Current, valid nursing license (Licensed Practical Nurse or Registered Nurse) through the Arizona State Board of Nursing
- Compact RN or LPN license
- Valid nursing licensure throughout the course of employment
- CPR certification at time of hire
- Fingerprint Clearance Card
- Minimum of three (3) years LPN experience in an LTC/SNF/Rehab setting
- Current RN licensure through the AZ State Board of Nursing AND have a minimum of one (1) year experience in an LTC/SNF/Rehab setting
Desired Qualifications
- Ability to obtain RN licensure through the AZ State Board of Nursing within six (6) months to one (1) year of hire
