Job Summary

Conduct comprehensive assessments with patients and community members to identify healthcare and social service needs, then develop individualized action plans to address barriers to care. Provide direct navigation support by facilitating applications for public benefits, healthcare coverage, and community resources while coordinating ongoing follow-up to ensure successful referrals. Collaborate with internal departments and external partners to maintain accurate documentation, client records, and resource databases in compliance with HIPAA and regulatory requirements. Participate in community outreach events and conduct training for staff and volunteers to strengthen service delivery. Maintain current knowledge of Pinellas County healthcare systems and public assistance programs to effectively serve clients.

  • Ensures communication and interactions with patients, clients, and community members are conducted with dignity and respect
  • Adheres to agency policies and procedures and all Federal and State regulations and local ordinances
  • Strong relationship-building skills with both internal and external stakeholders
  • Excellent communication and client engagement abilities
  • Strong problem-solving, critical thinking, and resource coordination skills
  • Ability to work effectively across departments and with community partners
  • Demonstrated ability to manage multiple priorities and follow through on client needs
  • Experience in maintaining accurate records and databases
  • Proficient in Microsoft Office Suite and related software
  • Ability to learn and effectively utilize a variety of technology platforms and software
  • Commitment to SPFC's mission and values, and a person-centered approach to service
  • Ability to meet deadlines
  • Minimum of two years of experience in social services, healthcare navigation, case management, or a related field
  • Must be able to lift up to 15 pounds at times
  • SPFC is a Drug and Alcohol-Free Workplace

Desired Qualifications

  • Knowledge of healthcare systems, public health benefits, and community resources preferred
  • Experience in conducting community outreach, engagement, or resource navigation preferred
  • Bachelor's degree in Social Work, Human Services, Public Health, Healthcare Administration, or a related field preferred
  • Knowledge of public assistance programs, healthcare coverage options, and community resources available throughout Pinellas County preferred
  • Bilingual (Spanish) strongly preferred
