Job Summary
Respond to all referrals in a timely manner by conducting strengths and needs assessments to connect youth and families to state and federal services. Provide ongoing case management, transition planning, and discharge/aftercare support while maintaining accurate documentation and reporting in compliance with HIPAA. Offer crisis intervention and skills training, including after-hours coverage during on-call rotations and holidays. Collaborate with co-located providers, referral agencies, and community organizations to establish formal partnerships and share resources.
Required Qualifications
- one year of case management or involvement in connecting families in federal or state service navigation such as WIC, TANF or regulated service system
- Understanding of crisis intervention and trauma informed care interventions
- good verbal and written communication skills
- Possess a working knowledge of basic computer skills
- Exposure to children/youth with behavior problems, including possible physical aggression
- Exposure to illness and blood
- Maintain confidentiality and follow policies related to HIPAA and control of client information and any personnel records
- Offer services after normal business hours, on holidays and during assigned on call rotation
- Complete training and certifications as required
Desired Qualifications
- A bachelor's degree in social work, psychology or related human services field from an accredited college or university
- An additional year of experience working with families and youth in case management/service coordination may be substituted for a year of college (30 hours) to secure a bachelor's degree
- Bilingual skills
