Job Summary
Support individuals in their home and community by completing activities of daily living and ensuring their needs are met. Monitor health, assist with transportation and medical appointments, and help manage finances. Implement supports outlined in individual service plans, maintain a clean and healthy environment, and complete all necessary documentation and action plans. Ensure the home is equipped safely and report any hazards or issues to the Program Manager. Maintain confidentiality and follow all agency policies while attending required training. Work flexible hours including weekends, evenings, and holidays.
Required Qualifications
- Must have a Nevada drivers license*
- High School education or equivalent
- Ability to read and write in English
- Criminal background check with no convictions
- Pre-employment Drug Testing
- Good organizational and time management skills
- Ability to self initiate responsibilities
- Two positive professional references and one positive personal reference
- Communicate effectively with individuals, families, coworker's and administration
- Ability to maintain confidentiality
- Ability to recognize and report hazards and apply safe work methods
- Mobility to work in a typical residential setting and use standard home appliances
- Exert force of 10-100 pounds, occasionally
- May involve climbing, balancing, stooping, kneeling, crouching, talking, reaching and using hands to grab objects at far reach
- May involve negligible to significant standing/walking/pushing/pulling/carrying
- Involves hearing and speech to communicate in person or over the phone and vision to read printed materials and view training videos
